This Is a copy of the Contract for hypnotherapy with Whole Hypnotherapy

CONTRACT 

This is a contractual agreement between you (the client) and I (Rachel Smith, the  therapist) for hypnotherapy sessions. Please carefully read through the following  points, I will be happy to answer any questions you may have.  

• I will respect your views, your beliefs, ideals and goals. I’m not here to tell  you right from wrong, I’m here to help you work towards your goals to the  benefit of you. I will treat you with respect and ask you do the same for me.  Your well-being is of the utmost importance to me. 

• Each standard session is charged at £60.00.  

• A single session will last approximately 50 minutes but the length of the  session can vary. The hypnosis itself will be between 25 and 35 minutes  depending on requirement. 

• If a longer session is required a 90 minute slot at £90.00 can be arranged. • Smoking cessation is approximately a two hour session and is £150.00. • All fees are payable on the day of the appointment by cash or card payment. • Additional fees will be charged if you wish to have a recording which has been created outside of our therapy sessions, for personal use, details of which are  available on request. 

• Invoices will be issued on request. 

• Concessional sessions are available with proof of low income. • All sessions will take place at the office address outlined above unless under  exceptional circumstances.  

• You acknowledge that I make no guarantees of success or cure. In order to get  the best results progress is collaborative, you should be a genuine participant  and fully engaged in the process. Part of this progress will happen through  reflection or homework outside of the therapy room. 

• You understand that hypnotherapy is not a replacement for medical diagnoses  and medical treatment from a qualified medical doctor. 

• You understand if I advise you to seek medical diagnoses and obtain a medical  doctor’s written consent to have hypnotherapy in order to continue with  treatment, you must do so before our next session. 

• If I advise you to seek medical diagnoses and a medical doctor’s written  consent to have hypnotherapy and you do not you then forfeit any pre-paid fees for further sessions until either written consent is obtained or a note advising  against hypnotherapy from a qualified medical doctor is produced (in this  instance all pre-paid fees for further sessions will be refunded by the original  method of payment). Either of these notes must be produced within two weeks of my request. 

• If you receive a recording it is for your personal use only and is under no  circumstances to be lent, copied, sold or played back to any one other than  yourself at any time. 

• It is not commonplace to receive a recording of a session.  

• If you receive a recording it should never be listened to while driving or  operating heavy machinery. 

• You commit to completing any homework you are given. 

• You agree to treat me and my place of work with respect. 

• You agree to attend sessions on time and sober, not under the influence of  alcohol or recreational drugs. 

• If you violate these standards or display any antisocial or aggressive behaviour  I reserve the right to end the session and or refuse any future sessions. Any advance payment will be forfeited. 

• Cancellation can only be made by email and requires a minimum of 48 hours notice or the full session fee will be charged. 

• Appointments booked less than 48 hours in advance cannot be cancelled or rescheduled without the full session fee being charged. 

• Cancellations made before the agreed 48 hours will be refunded via the  original payment method if not rescheduled. 

• If I need to cancel a session I will give as much notice as possible. If this is  less than 48 hours you will receive a full refund if you prefer not to reschedule. 

• If you are running late or anticipate running late the session cannot take place  if you will be more than 20 minutes late. If you are late the session will not be  extended and will finish at the scheduled time. 

• In between sessions any necessary contact will be by email, SMS or WhatsApp only. 

• Should we meet outside of therapy, in the nicest way possible, I will not  acknowledge you. I don’t intend it to be rude, it is to maintain your privacy and  confidentiality. 

• I have the right to end our sessions and refer you to other specialists if I feel  your case is outside that of my expertise. Any following pre-paid sessions will  be refunded by the original method of payment.

• I am a registered member of the National Hypnotherapy Society, which is the  only hypnotherapy organisation to hold a register accredited by the  Professional Standards Authority.  

• Any complaints you may have should be directed to the Professional Conduct  Officer on 01903 213683 during working hours or email

• Confidentiality is taken very seriously. Everything you say in session will be  under strict confidence and not shared without your express permission with  the following exceptions: 

If you indicate to me intentions of suicide or serious self harm it is my duty to  contact your GP. 

Any abuse or injury to a minor will be reported to the appropriate authorities. If there is good cause for me to believe that others could be in danger of  serious harm. 

The sharing of general non-specific information with any referring NHS  medical practitioner. 

Occasionally anonymous cases or case histories will be discussed with  supervisors or during Continuous Personal Development on my part, at no  time will any of your personal identifying details be shared. 

Occasionally it may be helpful to record your session in order for me to  prepare for future sessions with you or to discuss it with you or my supervisor.  It will only be done so with your written consent and you are under no  obligation to agree. If you choose not to do so your therapy will not be  affected. Should a recording take place it will be destroyed within 72 hours.  

• Any data and information collected from you will be securely and digitally  stored. It will be used solely for the purpose of your treatment.  

• Any and all copies of your case file will be destroyed one year after the end  date of your therapy.
